GCHCC launches community health assessment

Help improve the health of Green County


MONROE — The Green County Healthy Community Coalition (GCHCC) has launched the 2024 Green County Community Health Assessment. This is a process the coalition undertakes every three years to understand Green County’s strengths and areas for improvement related to health. The GCHCC is co-chaired by representatives from Green County Public Health and SSM Health Monroe and has representatives from over 50 different organizations across the county. The coalition kicked off this exciting initiative by hosting a meeting with coalition members on Thursday, May 2, at SSM Health Monroe. At this meeting, coalition members learned more about the assessment, timeline, and how they can participate.

As a part of the community health assessment process, the GCHCC has developed an online survey, which will be available until June 15, for community members to provide feedback about what they think the community’s strengths are and what health issues they see in the community. The survey takes 5-10 minutes, and people who complete the survey can enter for a chance to win a $25 gift card. In addition to the survey, the coalition will look at data from sources like the Wisconsin Department of Health Services, County Health Rankings & Roadmaps and others to gather secondary data on factors that contribute to health and health outcomes in Green County. Additionally, the coalition will be conducting interviews with community leaders and hosting community listening sessions to hear from community members. These conversations will help the coalition get input from lots of different people in Green County which will lead to a better understanding of different health issues facing the community. The results from the online survey, data gathering, stakeholder interviews, and community listening sessions will be analyzed to identify important themes and health issues. The coalition will use all of this information to determine which health priorities need to be focused on and develop the community health improvement plan. The health improvement plan will guide the coalition’s course for the next three years and beyond.

The last time the coalition conducted a community health assessment was in 2021. As a result of that assessment, the coalition selected three health priorities to focus on: Housing, Mental Health, and Adverse Childhood Experiences. Over the past three years, coalition members have been working together to improve health related to these priorities, including hosting the first Green County Housing Summit, bringing Parent Cafes to Green County, and providing numerous trainings and events focused on mental health.
