GREEN COUNTY — From now through March 14, a collaborative community health team will be offering $20 gas card incentives to all adults (18-plus) who complete their primary COVID-19 vaccine series or receive a booster dose of the COVID-19 vaccine in Green County.
The goal of this program is to empower local Green County residents to work with trusted messengers of health including public health, health systems, pharmacies, local businesses, schools, and community-based organizations to increase access to COVID-19 vaccinations and promote opportunities to boost health in Green County.
Take two safe, effective, and free steps to boost individual health and receive a $20 gas card incentive:
First, get vaccinated against COVID-19 or receive a booster dose at one of the following locations:
● SSM Health Monroe — 515 22nd Ave, Monroe
● Green County Public Health Department — N3152 Wis. 81, Monroe
● Walgreens — 717 8th Ave, Monroe
● Monroe Hometown Pharmacy West — 131 W 7th St, Monroe
● Walmart — 300 6th Ave W, Monroe
● Pinnow Hometown Pharmacy, call (608) 897-2595 — 1028 1st Center Ave, Brodhead
● New Glarus Hometown Pharmacy, call (608) 527-2517 — 1101 Wis. 69 #7, New Glarus
Second, when the primary COVID-19 vaccine series or get a booster dose is completed, a ticket with a link to a survey will be received. Complete the online survey (or fill out the identical survey on the back of the ticket) and exchange the ticket and proof of survey completion at one of the following businesses:
● New Glarus Culver’s - 1405 Wis. 69, New Glarus
● Veracruz Mexican Market - 500 14th Ave, Monroe
● Brodhead Piggly Wiggly - 1604 1st Center Ave, Brodhead
Individuals will be able to choose which COVID-19 vaccine or booster they receive. For detailed information regarding COVID-19 primary vaccines and boosters, visit The Green County Boost webpage.
Individuals do not need to have an ID, proof of residence, or health insurance to participate. The vaccine is completely free. People with health insurance, however, are encouraged to bring their insurance card so that insurance can be billed for administrative costs.
Support for The Green County Boost is made possible through a grant to the Wisconsin Community Health Empowerment Fund (WCHEF) and the Southwestern Wisconsin Community Action Program (SWCAP) by the Wisconsin Department of Health Services. The collaborative community health team includes staff from the Green County Public Health Department, the Monroe Clinic, SWCAP, the United Way of Green County, the UW-Extension Green County and the Wisconsin Community Health Empowerment fund.
