Beginning Jan. 25, all Wisconsin residents 65-years and older are eligible to receive the COVID-19 vaccination.

MONROE — Green County Public Health will begin hosting COVID-19 vaccine clinics for people 65 years of age and older. The first clinic will take place Feb. 1, and the second Feb. The department will be providing the Moderna COVID-19 vaccine.

Individuals 65 or older and are residents of Green County can make an appointment for the clinic at

gcpublichealth.org. The clinic will be held from 9 a.m. to 2 p.m. in the lower level multi-purpose rooms of the Green County Government Services Building (N3152 Hwy 81, Monroe, WI 53566). Vaccine availability and the department’s capacity to provide vaccine are limited — once the available vaccine appointment slots are filled, it will not be able to take more appointments until the next vaccine clinic.

The department suggests to review four information sheets for their knowledge and safety. These include: The Moderna Emergency Use Authorization Fact Sheet, a V-Safe Information Sheet, and the Next Steps After You’ve Received the Vaccine information sheet. Individuals should also review, print and fill out the GCPH Vaccine Administration Record and Screening Sheet and bring this form to their appointment. All of these documents are available on the department’s website.

Individuals will need to provide proof of age through a form of identification (ID) with their date of birth. Individuals who have Medicare or Medicaid should also bring their card to their appointment.

People 65 years of age and older without access to internet or a computer may have a family member or loved one with access to internet schedule an appointment for them. At this time department phone lines are extremely busy — for the convenience of community members and staff, the department strongly encourages everyone schedule appointment online. Those without internet access can call the department at 608-328-9390.

To ensure a safe and efficient process, please plan to take the following steps:

●  Arrive no more than 5 minutes before appointment time.

●  Bring Vaccine Administration Record and Screening Sheet, a form of ID with date of birth, and Medicare or Medicaid card

●  Wear a mask/cloth face covering

●  Plan to stay at the clinic for at least 15 minutes after receiving vaccine

The Moderna COVID-19 requires a second dose of the vaccine 28 days after the first dose. During the 15-minute waiting period after receiving the COVID-19 vaccine, a staff member will work assist setting up an appointment for the second dose — which will take place on Monday, March 1 or Wednesday, March 3.

As the vaccine becomes more widely available, Green County Public Health will take the appropriate steps to communicate with the public regarding how they can get vaccinated. The federal government, state of Wisconsin, and local vaccine providers are working to ensure the vaccine is rolled out in a fair and equitable manner through a phased approach. Patience of community members is greatly appreciated during the phased rollout.
