TOWN OF KENDALL — A 60-year-old Platteville man died in a two-vehicle crash on U.S. 151 just south of the Lafayette–Iowa county line Monday afternoon.

Howard A. Mutert was pronounced dead at the scene of the crash at 21990 U.S. 151 around 4:05 p.m.

A passenger in Mutert’s van, William O. Simba, 37, Platteville, was transported to Upland Hills Health in Dodgeville with serious injuries.

According to the Lafayette County Sheriff’s Office, Mutertpulled out of the driveway and into the path of a northbound car driven by Grace O. Pawlowski, 22, Waukesha. The Sheriff’s Office said Pawlowski was unable to avoid striking Mutert’svan.

Mutert’s van ended up in the median, and Pawlowski’s car ended up in the paved crossover.

Pawlowski was treated and released from Southwest Health in Platteville.

A second crash occurred near the crash scene due to slowed traffic.
