MONROE - The School District of Monroe and the Green County Public Health Department announce the dates for school-based H1N1 vaccination clinics. The Green County Public Health Department will give the H1N1 vaccinations (injection form only) on the following dates, times and schools:

• 8:30 to 10:30 a.m. Thursday, Dec. 3 - Northside School

• 7:30 to 11 a.m. Wednesday, Dec. 9 - Monroe High School (Virtual School invited)

• 12:45 to 3 p.m. Wednesday, Dec. 9 - Monroe Middle School (Charter School invited)

• 8:30 to 11 a.m. Friday, Dec. 11 - Abe Lincoln School

•12:45 to 2:15 p.m. Friday, Dec. 11 - Parkside School

All 4K and early childhood students are invited to attend any of the above school clinics, but must have a consent signed by a parent and must be accompanied by an adult.

Parents: If you already have sent a completed vaccine consent form for your child, they will be vaccinated on this date with the Injectable form of vaccine. Children under 10 years of age need two doses of the H1N1 vaccine separated by at least three weeks. For those children needing a second dosage, vaccination clinics will be at the elementary schools in early January.

If your child already received the first dose more than three weeks ago, contact Lori Soderberg, R.N., at 328-7143, as your child can be vaccinated with the second dose at the school based clinic. Also, if your child already has received the vaccine and a consent form was sent in to the school district, notify Soderberg.

If you did not sign a consent form for the vaccine for your child previously and have decided you would like them to have it, you may pick up a form at the school office or download it from the school Web site under School Health Corner at www.monroeschools.com. Send the signed consent form back to your child's school by Dec. 7 so your child can receive the vaccine at the school-based clinic. This is a voluntary vaccination, not required and is free of charge from the government.

If you would like to be present for your child's vaccination, you may do so, but stop at the school office first to check in. Times will vary for each class, so the school won't know ahead of time what time children will be vaccinated.

For more information, call Soderberg, district nurse for Monroe Schools, or the Green County Public Health Department at 328-9390.
