Monroe Clinic pharmacy initiative earns high praise

MONROE - A new Monroe Clinic initiative that expands the pharmacist's role in chronic disease management is garnering recognition throughout the region. The new initiative recently earned second place at Concordia Medication Management Accelerator's pitch competition in Milwaukee, where Monroe Clinic competed against 12 other pharmacist-led teams - including teams from Aurora Healthcare and Walgreens.

Formed as part of Monroe Clinic's Collaborative Care Model, the program begins as physicians identify patients that can benefit from follow-up medication consultations with Monroe Clinic pharmacists. Chronic conditions require in-depth, complex medication management. Individuals with diabetes, depression/anxiety or thyroid, blood pressure or cholesterol issues will benefit from this unique service.

This new program is designed to provide cost-efficient support, an optimized medication plan and ongoing outreach to help patients reach their health management goals.

The program also was the inspiration for a poster campaign, which received second place at the Rural Community Poster Fair hosted by Wisconsin Collaborative for Rural Graduate Medical Education held at Minhas Brewery.
