MONROE — The Green County Coroner’s Office reported the name of the third victim of a fatal crash that occurred on Oct. 9, 2021 outside of Brodhead. 

At approximately 7 a.m. on Oct. 9, at the intersection of Wis. 11/81 and County GG, a vehicle vs. semi-truck crash resulted in three fatalities. Two victims were identified in the days following the crash: Juan Carlos Lopez Velaquez, 42, and Gustavo Adolfo Martinez, 33. On Dec. 20, the third victim was identified as Evaristo Antonio Perez, 51 of Beloit. 

Velaquez, the driver, failed to stop at the stop sign on County GG and collided with the eastbound semi-truck on Wis. 11.

According to a Green County Sheriff’s Office press release Oct. 9, “Fog, speed, and the traffic violation are all factors in the crash.”
