REQUEST FOR QUALIFICATIONS - Pleasant View Nursing Home Building Renovation

The Pleasant View Nursing, owned and operated by Green County, is seeking proposals for services relating to the design of building renovation relating to window replacement, siding and construction/renovation of bathrooms and to include securing necessary permits, assist with bidding process and construction management. Qualifications will be taken for professional services to assist Green County with this project. The project is funded with Community Development Block Grant Funds which have been awarded to Green County. The selected architect/engineering firm will provide professional services to develop detailed plans of the proposed renovations enabling the County to secure the necessary permits to proceed with the project; the development of bid specifications; assisting the County with the bidding process and providing oversight of the construction phase of the project. A rating system will be utilized for selection of the architect/engineering firm. This rating system will be employed by Green County in determining which proposal best meet the needs of the County and the renovation project. The rating system take into consideration the following criteria: 1. Firms Experience. Briefly identify the firms experience with major building renovation projects which incorporates accessibility and energy efficiency standards and cost estimates. Identify 2 somewhat recent projects similar in nature to our proposed project. 2. Professional Qualifications. Your criteria and design approach to be used in the performance of required work. The personnel to be assigned to the project along with their qualifications and experience. 3. Firms familiarity with nursing home or assisted living facility projects. Identify your firms work with smaller/rural communities (populations of less than 10,000). Qualifications must be received no later than 1:00 PM, Monday, November 2, 2020 at the Pleasant View Nursing Home Office, N3150 State Highway 81, Monroe, WI 53566. Information for the planned bridge replacement located in Section 21, T02N, R06E, Town of Jordan, Green County, will be available for public review and comment beginning on Wednesday, October 28, 2020, via the Green County Highway Department’s website, the Green County Highway Department announced today. Project information regarding the bridge replacement on CTH Y will be made available by 5:00 PM on Wednesday, October 28th via the Clark County Highway Department’s website located at: http://www.co.green.wi.gov/localgov_departments_details.asp?deptid=112&locid=148. The Green County Highway Department is proposing to replace an deficient bridge (P-23-0150) that carries CTH Y over Jordan Creek. The project is located approximately 150 feet East of the junction of CTH M / CTH Y. The project will involve a bridge replacement and reconstruction of approximately 270’ of approaches. Green County is proposing to close CTH Y to through traffic during construction operations. No detour will be posted. Alternate routes are available.
