Advertisement for Bids Pleasant View Nursing Home Facility Renovation Monroe, Green County, Wisconsin

Bid Opening: Thursday, May 27, 2021 at 1:00 p.m., C.S.T. The Pleasant View Nursing Home will receive sealed bids via the online electronic bid service through QuestCDN VBid online bidding or by sealed paper copy. Sealed paper bids shall be delivered to the Pleasant View Nursing Home located at N3150 State Hwy 81, Monroe, WI 53566 for the Pleasant View Nursing Home Facility Renovation. Both electronic and paper bids will be publicly opened and read aloud on Thursday, May 27, 2021 at 1:00 p.m., C.S.T. Description of Work: This Project consists of full window replacements and exterior metal panel installation, as well as existing restroom renovations and the construction of accessible restrooms. Work includes, but is not limited to, all demolition, framing, finishes, electrical, HVAC and plumbing. Bids shall be on a lump sum price basis with Alternates. Bidding Documents will be made available to Prospective Bidders by Monday, May 3, 2021. Printed Bidding Documents may be obtained from the Issuing Office during the hours indicated above for a non-refundable deposit of $145.00, including shipping, upon Issuing Office’s receipt of payment for the Bidding Documents. Consideration of Bids: No proposal will be accepted unless accompanied by a certified check or bid bond equal to at least five percent (5%) of the amount bid, payable to the OWNER as a guarantee that, if the bid is accepted, the Bidder will execute and file the proper contract and bond within 15 days after the award of the contract. The certified check or bid bond will be returned to the Bidder as soon as the contract is signed. If after 15 days the Bidder shall fail to do so, the certified check or bid bond shall be forfeited to the OWNER as liquidated damages. The OWNER reserves the right to waive any informalities, or to reject any or all bids, or any part of a bid, and to award the Contract to the Bidder, who in the judgment of the OWNER, will best serve the interest of the OWNER. Additional Conditions: The letting of the work described herein is subject to the provisions of Sections 62.15, 66.0901, and 66.0903(1m) of the Wisconsin Statutes. This project is funded in part by a Community Development Block Grant CLOSE for Public Facilities (CDBG CL-PF) from the Wisconsin Department of Administration using federal CDBG funds. Wage Rates: The Contract shall be subject to Section 66.093 of the Wisconsin Statutes pertaining to municipal wage scales and Federal Davis-Bacon wage rates. Attention of bidders is particularly called to the requirements as to conditions of employment to be observed and minimum wage rates to be paid under contract, Section 3, Segregated Facility, Section 109 and E.O. 11246. Any contract or contracts awarded under this Advertisement for Bids may be funded in part by a grant from Community Development Block Grant Program. Minimum salaries and fringes to be paid on the Project shall be in accordance with the prevailing wage rate determination established by the Federal Department of Labor (DOL). Attention is also called to the fact that not less than the minimum salaries and fringes set forth in the Contract Documents must be paid on this Project, and that the Contractor must ensure that employees and applications for employment are not discriminated against because of their race, color, religion, sex, national origin, or other protected class. Federal Labor Standards program laws, including, but not limited to, the Davis-Bacon Act, the Copeland Anti-Kickback Act, and the Contract Work Hours and Safety Standards Act will apply to this Project. Objections to the amount or valuation of property shall first be made in writing and filed with the clerk of the board of review within the first 2 hours of the board’s first scheduled meeting, except that, upon evidence of extraordinary circumstances, the board may waive that requirement up to the end of the 5th day of the session or up to the end of the final day of the session if the session is less than 5 days. The board may require objections to the amount or valuation of property to be submitted on forms approved by the Department of Revenue, and the board shall require that any forms include stated valuations of the property in question. Persons who own land and improvements to that land may object to the aggregate valuation of that land and improvements to that land, but no person who owns land improvements to that land and improvements to that land may object only to the valuation of that land or only to the valuation of improvements to that land. No person may be allowed in any action or proceedings to question the amount or valuation of property unless the written objection has been filed and that person in good faith presented the evidence to the board in support of the objections and made full disclosure before the board, under oath, of all of that person’s property liable to assessment in the district and the value of that property. The requirement that objections be in writing may be waived by express action of the board. 5. When appearing before the board of review, the objecting person shall specify in writing the person’s estimate of the value of the land and of the improvements that are the subject of the person’s objection and specify the information that the person used to arrive at that estimate. 6. No person may appear before the board of review, testify to the board by telephone, or object to a valuation if that valuation was made by the assessor or the objector using the income method of valuation, unless the person supplies the assessor with all the information about income and expenses, as specified in the assessor’s manual under s. 73.03 (2a), Wis. Stats., that the assessor requests. The City of Monroe has an ordinance for the confidentiality of information about income and expenses that is provided to the assessor under this paragraph that provides exceptions for persons using information in the discharge of duties imposed by law or the duties of their officer or by order of a court.* The information that is provided under this paragraph, unless a court determined that it was inaccurate, is not subject to the right of inspection and copying under s. 19.35 (1), Wis. Stats. 7. The board shall hear upon oath, by telephone, all ill or disabled persons who present to the board a letter from a physician, surgeon, or osteopath that confirms their illness or disability. No other persons may testify by telephone unless the Board, in its discretion, has determined to grant a property owner’s or their representative’s request to testify under oath by telephone or written statement. 8. No person may appear before the board of review, testify to the board by telephone, or contest the amount of any assessment unless, at least 48 hours before the first meeting of the board, or at least 48 hours before the objection is heard if the objection is allowed under s. 70.47 (3) (a), Wis. Stats., that person provides to the clerk of the board of review notice as to whether the person will ask for the removal of a member of the board of review, and, if so, which member, and provides a reasonable estimate of the length of time the hearing will take.
