Group seeking GC caregiver opinions

MONROE - The newly formed Green County Caregiver Coalition is looking for experiences and opinions from Green County family caregivers to help fulfill its mission to support, educate and advocate for those who give and receive care.

The coalition is offering a survey that allows them to gather information to improve services and programs.

The survey is anonymous and there are three ways to participate - online at www.surveymonkey.com/s/R3HKMTV; receive a paper copy by calling (608) 328-9499; or call in to (608) 328-9499 and take the survey.

Green County Caregiver Coalition members include the Aging and Disability Resource Center, Alzheimer's and Dementia Alliance of Wisconsin, Angelus Retirement Community, Behring Senior Center, Comfort Keepers, Community Volunteers, Green County Health Department, Harbor House, IRIS, Lena Living Center, Monroe Clinic, Monroe Manor, New Glarus Home, Pleasant View Nursing Home, Senior Behavioral Sciences, Southwest Health Center, Southwest Family Care Alliance, St. Clare Friedensheim, Swiss United Church of Christ, UW-Extension and Wood's Crossing.
